I have a question about the translation of the first line:

Where does the "never" come from? It seems to say he knew a woman like her.

the translation is correct. The word "never" is inferred in the sentence.It is similar to saying "in your life you'll see someone so beautiful like her".

I trust you guys, but wouldn´t it make much more sense for the line to be interpreted as; "Once in my life I knew a woman they called `la flaca`"?
I kinda took "igual a la flaca" to mean that instead.
